Step 1: Detailed Questionnaire
Everything begins with a detailed questionnaire about your loved one. It’s important to be as detailed as possible so our team can have a detailed plan in mind before our virtual meeting.
Our Process
Our process is simple, thoughtful, and designed to truly understand each person living with dementia. We focus on dignity and respect to help your family find the best path forward.
Step 2: Consultation
Next, is our virtual consultation where we review the details you have shared, come up with an action plan, ask any follow up questions, and overall have a safe space to talk about what you are dealing with with a dementia care specialist.
After our meeting, this is where the personalized Independence Kit gets created. The Independence Kit is optional for those that do a consultation. All the information is collected from the questionnaire and from our call gets annalized and turned into a kit to keep your loved one independent at home.
